Self-Pay Direct Access

Same-day or same-week availablity for consults or procedures. Skip the insurance delays and denials with direct access to specialty care for enhanced and concierge services — please inquire for details.

Commercial Insurance Carriers

  • Aetna

    • PPO only; no referral needed

  • AvMed

    • Some plans allow out-of-network benefits

  • Blue Cross Blue Shield (BCBS)

    • HMO/PPO; not in-network with Select. HMO plans require a PCP referral.

  • Cigna

    • PPO only; not in-network with HMO plans.

  • First Health

    • No referral required; most plans do not cover surgical procedures.

  • Medicare, Original Part B

    • Accepted; no referral required.

  • UnitedHealthcare (UHC)

    • HMO/PPO; some HMO plans require a PCP referral

Referrals & Authorizations

  • HMO plans often require a referral from your primary care provider (PCP).

  • Prior authorization may be required for certain services; coverage varies by plan.

  • Verification of benefits is not a guarantee of payment; final responsibility is determined by your insurer.

Workers’ Compensation

All workers’ compensation cases are accepted. Approval from the claims adjuster is required.

Personal Injury

We accept legal cases and all automobile insurance. Please provide your attorney and PIP (Personal Injury Protection) information.
